Central nervous system melioidosis is a rare but severe manifestation of Burkholderia pseudomallei infection. We present a case of a 74-year-old man with type 2 diabetes mellitus who developed fever, headache, and progressive neurological deficits. Imaging revealed a right frontoparietal brain abscess, scalp collection, and osteomyelitis. B. pseudomallei was isolated from the blood and tissue cultures.
