A 53-year-old obese female patient weighing 132 kg, presented persistent dyspnea lasting one week and came to the hospital to perform a follow-up thoracic CT scan. The patient was diagnosed with granuloma two years ago and had no prior history of other interstitial or vascular lung disease. The CT scan revealed an incidental finding of Pulmonary Embolism (PE) in the right pulmonary artery and its segmental branches. Despite the patient?s significant body weight, the use of 50 mL of contrast media was enough to provide image quality for diagnosis. The patient was subsequently hospitalized and treated with anticoagulants, showing improvement before discharge. This case highlights the capability of advanced CT technology with an energy-integrating detector (EID) in managing complex cases with minimal contrast exposure.
