Backgtound & Purpose: The benefit of evidence-based medicine is to avoid the shortcomings of clinical care that relies on local expert opinion and to provide a framework for evaluating and using the latest available evidence in decision making. We would like to demonstrate the importance of clinical guidelines and protocols in an acute and chronic disease, namely acute pancreatitis (AP) and cystic fibrosis (CF). ln this papef we have summarized the results of two registry analyses on the importance of protocolized patient discharge and psychological intervention in patients with alcohol-induced AP We highlighted the consequences of non-adherence t0 guidelines through a case report and evaluated the association of dilferent BMI categories with clinical outcomes in CF through a systematic review and meta-analysis. Methods: A multicenter, web-based survey was conducted to assess the worldwide trends in the discharge of patients with AP For the two cohort analyses, data on the Hungarian AP population were collected from the Hungarian Pancreatic Study Group (HPSG) prospective registry. For the meta-analysis, a systematic search was performed and the pooled results (p0R and pMD) were presented in forest plots. Results: Only a few centers (7156, 12.5V,) use a protocol when discharging a patient with AP Patients discharged according to the protocol had a significantly shorter hospital stay - 7 (5; 10); I (5;12) days - and the risk of readmission was not higher. ln-hospital patient education significantly reduced post-discharge alcohol consumption as measured by GGT and self-report (294.00-r250.75 vs. 103.20+113.50 UIL;80"/" remained abstinent). Higherthan normal BMlwas associated with more favorable clinical outcomes in CF patients. Conclusion: AP protocols can significantly reduce hospital length of stay without increasing the risk of readmission. lncorporating Bls into regular inpatient and outpatient care could promote abstinence and prevent relapses.
