The management of chronic pediatric crohn disease is a challenge for all pediatric and adult gastroenterologists. Physicians must consider the long-term advantages and disadvantages of each intervention. 0ver the past few decades, advances in the therapeutic armamentarium have fundamentally changeo ttre goat of treatment. current guidelines recommend a treat-to-target approach with the goal of mucosal healing. Today, all available medications locus on inflammation control with significant potential adverse events. However, a 6-8 week period of exclusive enteral nutrition has been shown to be effective in inducing remission in children with crohn disease. This observation is not only critical to reducing the use of corticosteroids in pediatric crohn disease, but also highlights the importance of diet in the development of in{lammatory bowel disease. As diet is recognized as one of the major triggers in crohn disease, identifying an ideal diet could potentially serve as a causal treatment for patients whose disease is largely influenced by their diet.
